Job Description:

compassionate care. Currently, we are seeking a skilled Social
Worker to join our team. The successful applicant will be
instrumental in planning, coordinating, and providing social work
services for patients and families, addressing psychosocial needs
and enhancing psychological functioning. This role requires a
profound commitment to enhancing patient care, impeccable ethical
standards, and the ability to thrive in a multidisciplinary team
environment. Conduct patient assessments to determine their
emotional, social, and financial needs. Develop and implement
tailored care plans based on patient assessments. Provide
counseling and crisis intervention to patients and their families.
Facilitate family/patient meetings to ensure the well-being of
patients during their care journey. Coordinate with healthcare
professionals to enhance patient treatment with a focus on social
health. Advocate for patient needs within the facility and in
broader community resources. Continually assess patient progress
and revise care plans as necessary. Strong interpersonal and
communication skills Ability to handle highly sensitive situations
with compassion and professionalism Knowledge of healthcare policy
and relevant social healthcare systems Crisis intervention
techniques Case management proficiency Excellent documentation and
