Job Description:

Encompass Health Corporation is seeking a dedicated Case Manager to join our multidisciplinary healthcare team. This position will focus on facilitating effective healthcare outcomes for our patients across inpatient rehabilitation, home health, and hospice settings. Our Case Managers play a crucial role in improving patient care by assessing needs, planning and coordinating treatment plans, and monitoring patient progress. Ideal candidates should have a strong understanding of healthcare protocols, interdisciplinary communication skills, and the compassion needed to assist patients through their healthcare journeys. Assess patient needs and develop individualized care plans in collaboration with healthcare professionals. Coordinate with medical, nursing, and accounting staff to ensure appropriate patient services are provided. Act as a liaison between patients, family members, and healthcare organizations to facilitate communication and ensure patients understand their treatments. Monitor patient progress and adapt care plans as necessary. Ensure compliance with healthcare regulations and maintain accurate patient records. Educate patients and families about health status, treatment options, and community resources. Strong interpersonal and communication skills Proficient in healthcare management systems Ability to work collaboratively in a healthcare team Strong organizational and problem-solving skills Knowledge of healthcare laws and regulations Medical Insurance Dental Insurance Vision Insurance 401k Licensed Registered Nurse (RN) or Certified Social Worker (CSW), preferred Case Management Certification (CCM), highly desirable
