Job Description:

A well-established geriatric care program in Mishawaka, IN is seeking a Medical Director with board certification in Internal Medicine or Family Medicine and fellowship training or certification in Geriatric Medicine . This role provides medical leadership for a Program of All-Inclusive Care for the Elderly (PACE) and ensures high-quality, participant-centered care for older adults. Key Responsibilities: Provide medical direction and supervision of all participant care and clinical outcomes. Oversee Quality Assurance Performance Improvement (QAPI) program. Develop and maintain medical policies, procedures, standards, and protocols. Supervise and provide guidance to PCPs and Advanced Practice Providers (APPs). Lead and collaborate with interdisciplinary teams and community providers. Conduct histories and physical exams, develop treatment plans, and perform periodic reassessments. Educate participants, families, and staff on chronic disease management and health maintenance. Participate in provider on-call rotation as required. Travel within the region periodically as needed.
