Job Description:
Job Title: Registered Nurse Profession: Nursing Specialty: Home
Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours
(08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health
Experience or 1 full year Hospice experience depending on the
specialty. License: Must have an active nursing license.
Certifications: CPR, BLS Must-Have: - Proficient in HomeCare
HomeBase (HCHB) and OASIS. - Must have recent specialty experience.
- Must have reliable transportation, a current driver's license,
and required liability insurance. Description: Provides,
coordinates, and directs the provision of home nursing care
according to physician's orders based on agency policies and
procedures. Responsible for the competent application of the
nursing process. Supervises and evaluates Home Health Aides and
Licensed Practical Nurses. Conducts home visits and participates in
periodic office meetings. Must be willing to travel up to a
specified mileage radius. Traveling is required to ensure timely
patient care and attendance at meetings. Average number of patients
seen daily is 5-8. Types of cases include Start of Care, Wound
Care, and Follow-up Visits. The contractor will be assigned between
5-10 patients per day, equating to roughly 6-8 points.
Documentation must be completed within 24 hours of patient
appointments. It is essential to maintain a high level of
productivity, meeting the weekly expectation of 30 points.
Declining patients may result in cancellation of the contract.
There are no guaranteed hours; however, options may be available to
float to nearby clinics if there are not enough patients. The
candidate must understand that travel may involve drives longer
than 20 minutes between patients. All applicants must be prepared
to accept every patient on their schedule.
