We hire and value those aligned and engaged with our mission, vision, and values, provide clear direction and opportunities, and empower them to reach their potential.Responsibilities:* Oversee the implementation and ongoing assessment of the patients plan of care* Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.* Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.* Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.* Actively participate in IDG meetings.* Provide supervision of aides and nurses.What we offer* Company Fleet Vehicles - (complete with a gas card, maintenance repairs and inspections, and insurance coverage)* Continued education programs to become certified in Palliative/Hospice care* Competitive Compensation* Full benefit packages* 401K* Flexible schedules* A united team approach focusing on high quality careRequirements* Licensure: Current license to practice as a Registered Nurse (RN) in the State of Pa.* Current CPR/FA certification.* Valid state-issued drivers license, automobile insurance, and reliable transportation; willingness to travel.* One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.*Advanced is willing to provide Employees with three (3) options in relation to a Sign-On Bonus. Those being a one-year, two-year and three-year option.
